Add online customised form

Hi Team,

We need a new feature added to customise online forms, we collect additional information from patients and would like to move them to online form to reduce the paper usage.

This is important for us so your speedy response would be appreciated, I know this is currently not possible in core, but this feature is becoming important for us, and we would like this implemented in the next couple of months.

I am attaching a sample form for reference.

  I second this! At the moment our clinic is having to use a completely diff online form.. then save as attachments in Core Practice. Having ability to customise online forms to suit individual needs would definitely be good!
