Improvement: CPITN score in the Charting feature

1. We need a way to show the CPITN score in "Charting". It would be useful to allow users to select when the CPITN input window appears. I suggest adding it in the "More" dropdown menu from the top of the chart:

It could also appear under the "Setting" heading, where it would be titled "Enter CPITN score".

2. This will then open a modal window (similar to the one to select a tooth number):

3. The data should be stored as a tab:

This is because there should be a log of which provider performed the diagnostic, the values and the date and time that this was performed. This tab may be hidden if the patient in question has not had a CPITN value recorded, or it could appear as "CPITN record (0)".
