Enhance Online consent form fields

Daniel from Dental Boutique said he really likes our form-building tool and that it is very easy to use. He would like to pass these enhancement recommendations in the future for the online consent form field to the development team:

• Page Breaks: Please introduce page breaks. These forms may not be printed, but they are saved as PDFs, and it is quite cumbersome to manually create page breaks.
• Fonts: more flexibility here would be great (sizes, colours)
• Tick/Check Boxes: Please consider adding them in
• Headers/Footers: Please consider adding them in
• Images: Currently, I'm re-using the Letter Head content part to add images, but the design options are poor, resulting in padding that I can't amend.
