Medical History Form

Hi Kenny,

When we click on Forms > Complete a New Form in the patient tab, the obsolete COVID-19 forms are appearing first, while the Medical History forms are pushed onto page 2.

I have tried switching the order from ascending to descending multiple times, logging out and back in, and checking again, but the order remains the same. This is causing some issues, as the team are accidentally selecting and sending the COVID-19 Clearance form out of habit, assuming it is the Medical History form.

Could you please advise if there is a way to remove or hide the obsolete COVID-19 forms completely so they no longer appear in this list?

Also, can you please let me know whether we are able to update the existing Medical History form or add a new Body Dysmorphia Questionnaire? I cannot find where to edit or create these forms, so I am assuming these settings may be system-generated or not editable from our end.
