Export treatment history & notes

I need the ability to export treatment notes, as this is something which health funds consistently need when doing audits (if this ever occurs). Also, some patients request their treatment notes after transferring to another dental clinic.

EDIT: Because these notes would be sent to health funds or another dental clinic, we need to remove internal comments from the export. This is because these notes are usually related to patient behaviour and not to their treatment history (which is what other clinics or health funds are interested in).
