New feature: Practitioner-specific performance dashboard

As submitted by OCM:

"I need to allow my practitioners to see their performance dashboard for a single day. The reason for this is because I want to have one place where they can go to check their daily totals, as opposed to my staff having to send it to them (since practitioners in my environment do not have access to reporting)."
