Kiosk/Digital Form: New Patient form via Kiosk
AnsweredWe get a lot of new patients and it would be great if we could use the Kiosk feature to send a link to them in an email so that they could pre-fill in a new patient form that populates all the information on their patients details. They could just use the sign in feature that links them to their patient profile and it could save some time before they come in to the surgery and also reduces the need for paper forms to be filled in. If you are also implementing signing features they could even sign the form as well. It could be integrated at the same time as updating their medical history form feature that you are bringing in?

Hi David,
I believe this capability is already available. With Core Practice Online Form you can have the follow process to request patient to complete the form:
- Automaticaly includes links in Appointment Email Notification which get sent to patient when appointment get created
- Manually Email or SMS link to request patient to complete the form 1 or sasme day before appointment
- Send SMS link to patient when arrive for appointment
- Cast form to iPad to request patient to complete the form

Hi David,
You are right about this one. We are absolutely aiming for a paperless environment so it makes sense that if we are successful with the digital medical history form then other forms can be digitised as well.
I will have access to the beta soon for testing. When I have gone through it on my own, I will be able to have select community members trying out the form and providing feedback. Would you be open to trying this out in your surgery?
Regards,
Warren
