Contact Management: Automatically add patients into contact list

This is in relation to 'Referral Details' report feature - https://support.corepractice.com.au/hc/en-us/community/posts/115007252247-Report-New-patient-referral-source-report 

Since we have this feature now, there should be an automatic population of patient contact list without needing to manually create them ourselves. 

This will help lessen the amount of 'Unspecified' referral sources.
