Appointment Book: Use Treatment Plan title as a reason for the appointment

When a patient has active or accepted treatment plans, we can currently add them into the appointment. However, there should be a way to add titles to the appointment so there is a better way for the team and patient to know exactly what will be done. 

The reason why we haven't done this yet is that when you use our reporting we have a report on Appointment reasons and this will ultimately mean that over time you may face duplicates with the same meaning, but worded differently. We suggest that you create enough reasons that'll cater to the treatment plans you typically do.
