Patient File: Add section for GP details

There isn't a specific section to add GP details right now. 

The purpose behind this improvement is to accommodate for the GP details in the correct category under Medical (as the pt has Dental and Medical category for History). Some Clinics are required to have the GP details on file (if pt has one).
