Improvement: Include Treatment Plan Names in PDF and Email

If a treatment plan is given a name, the name should appear on top of the treatment plan generated/printed/emailed - at the moment it just has 'the proposed treatment', furthermore, the pdf sent to the client should also have at least the clients name as the title of the pdf or 'clients name; date; treatment plan' etc.
