Improvement: Include Treatment Plan details missing in PDF output

When printing/emailing/downloading the treatment plan it would be good if the exported document had the Description of the visit and Time allowed like it shows on the screen. We go to the effort of adding it on the online screen only to find that when we give it to the patient, these details are not there so we need to manually write it down for them.

ie. Visit 1   Fillings top right   60mins   3 treatments   $xxx.xx

Items in bold are currently on screen but not on the exported document.
