Add a 'quantity' option when adding item codes to Treatments or Plan

As an example, I might do 2, 3 or 4 radiographs in one visit. Rather than adding 022 four times on four separate rows for one visit, I would like there to be a 'quanity' option when you enter an item code. There are a few other examples of item codes that are not tooth-specific, where you might do multiples in the same visit such as per arch or per quadrant codes.
