Add an 'excluding' feature to campaign/recall runs.

We want to target patients who have not had an 012 since pre-Covid, but as far as we can tell there is no way to filter patients who have NOT attended in a certain date range, or to generate a list excluding patients who HAVE attended in the last couple of years.
