Personalise/ Edit - Medical History Questions

Hi Team, our providers would really benifit from being able to edit / add medical questions such as:

Vaping (not just smoking)

and more detailed questions such as: 

Do you have any history of vitamin/mineral deficiency (Iron, Vit-B12, Folate, Vit-D, Calcium, etc.) or any history of vitamin/mineral supplementation?

Osteoporosis (If yes please be sure to list any medications below - eg. Prolia)
