How to Approve Online Medical History Forms

Overview

It is important for the clinic to keep the patient's medical history as well as to update it if necessary. If you are using the Core Practice online patient forms, here's how you can approve the medical history forms:

How To Approve an Online Patient Medical History Form
 

1. Go to Patient's Profile and click on Overview.

2. Below the Waiting Approve Form notification, click on the "Pencil Icon" to Review and Approve the Submission.
 

A. Click on Continue to History to proceed with saving the changes to the patient's medical history form.

B. Click Approve and Save the new patient medical history form.

NOTE: You can click Approve and Save to approve this submission by updating the below-listed changes to the system or click Approve without Save only approve but disregard the changes.


For Timestamp Purposes
Simply go to the Patient's Profile, and below the waiting approve form notification, click on the "Pencil Icon" and choose either View Evidence or View Signed Document. You can also view the Previous Request if there is any.
