Overview
A Hide condition allows you to show a follow-up field only when it is needed.
For example:
- The form asks, Do you have a Private Health Fund?
- If the patient selects Yes, the health fund details field appears.
- If the patient selects No, the health fund details field stays hidden.
Hide conditions use a Field key to connect the first question to the follow-up field.
What is a Field key?
A Field key is the unique system name automatically given to each field in Forms Builder.
You cannot change the Field key, but you can copy it when creating a Hide condition.
To find a Field key:
- Select the field you want to use.
- Hover your mouse to the choices, then click the pencil icon.
- Find and copy the Field key.
Add a follow-up question
In this example, the insurance details field should only appear when the patient selects Yes.
Step 1: Create or add the first question
- Add a field.
- Select the pencil icon.
- Enter the question, such as Do you have a Private Health Fund?
- Add Yes and No as the answer options.
- Copy the Field key.
- Select Save.
Step 2: Add the follow-up field
- Add the field you want to show, such as Name of the Health Fund, Membership No., ID# on card
- Select the pencil icon.
- Find the Hide condition field.
- Enter:
model.<field_key> != 'Yes'
Replace <field_key> with the Field key you copied.
For example: model.HealthFund!= 'Yes'
- Click Save.
The follow-up field will now appear when the patient selects Yes and stay hidden when they select No.
Important: Add the Hide condition to the follow-up field, not the first question.
Show more than one follow-up field
To show several fields after the same answer, add the same Hide condition to each follow-up field or section.
For example, you may want to show:
- Fund name
- Membership number
- ID# on card
Save and test the form
After adding the Hide condition:
- Select Save Changes.
- Publish the form.
- Test both answer options.
- Confirm the follow-up fields appear for Yes and stay hidden for No.
If a follow-up field does not appear, check that you copied the correct Field key and that the answer in the Hide condition matches Yes exactly.
