Can we suggest the option to being able to edit the medical history form so that includes all of the information we would personally like to collect from our patients in clinic.

  • How does this feature improve collection of patient data?
  • Are there compliance or documentation requirements specific to your clinic that this would address?
  • What type of patient information would likely be added to the form?

  Any additional examples you can provide would be extremely helpful as we evaluate this request.

  Yes that would be great. We currently don't use this feature because we're unable to customise the form.
