Improvement: Edit comments in a patient's Activities Feed

As a user, I should be able to edit comments which I have made in a patient's activities feed. This would require an audit log, tracking the changes made to each comment. This is important to me because I do not like grammatical errors and I need a way to edit a comment which I have made.

  Have added ability to flag that comment have been deleted instead to maintain integrity for patient's health records
