Charting: Add split occlusal and decoronation

It would be very helpful if the following additions could be made to the charting options:

1. Split occlusal surface into two sections (see red in PNG) - often the entire occlusal surface is not treated or two different materials have been used on the occlusal surface and only one can be charted.

2. Add 'decoronation' as a condition option (see purple in PNG) - this is where the tooth has broken off at the gum line but the roots are still intact, it is very difficult to display in charting with the current options.
