Overview
Consent forms help ensure compliance with legal and ethical standards, protecting both the rights of the individual and the organization providing the service. They enable individuals to make an informed decision about their participation, allowing them to understand the potential risks and benefits involved.
As a pre-requisite, we may suggest creating your consent form according to the service you're providing. You can click here for the step-by-step process of creating a consent form template.
Follow the steps below on how to email a consent form to a patient:
Steps
1. From the Patient Profile, go to the Letter tab.
2. Click on Create Letter button.
3. Select Location and choose the Provider of this letter before completing the template. Click on Continue to Template and select your chosen consent form template. Note: Use Optional Referred by or Referred to contact to enable referral mail merge.
4. Click on the Email button to send Patient Copy.
Note: E-signature for consent form is not yet available in Core Practice. The patient can use other external parties to sign the PDF consent form. Otherwise, Print or Download the consent form if the signature of the patient is required.
