Overview
We understand that treatment notes are very rarely the same between two practitioners. Everyone has their preferred styles, just as they have their preferred clinical techniques. Our treatment note templates are completely customisable, allowing you to create your own unique templates suited to your style. You've learned how to customise your treatment note templates, so now you can learn how to make the most out of your note templates. This article will teach you how to do the following:
- Make a question required
- Hide a question and make it appear once a condition has been met
- Create a placeholder for the question
- Enter a description for the question
Please start each section from the chosen template in the Quick Notes menu. You can find that by:
- Clicking on the Cog button and then on Quick Notes.
If you don't see this "Cog button" icon, you may not have the required permissions to edit your treatment note templates; please contact your administrator.
- Clicking on the Collection which you wish to customise.
- Click on the Template which you wish to edit.
In these examples, I will be making changes to my "Check up And Clean" template.
Make a question required
This option will make this question a required field. If a required question is not answered, the "Add Note" button will be inactive i.e. you cannot proceed until this question has been answered. Here's how to do it.
- Click on Option on the desired question from your chosen template.
In this example, I want to make "Reason for attending" a required question.
- Check the Must provide answer! box.
- Click Complete Template to save and exit.
a) Preview: gives you an opportunity to view your template before saving.
b) Save: allows you to save your progress and continue editing.
Hide a question and make it appear once a condition has been met
This option allows you to hide a particular question until a condition has been met with another question. For example, I may want to hide the "PBWs diagnosis notes" question if didn't answer the previous question "PBWs taken" or if I answered, "Patient declined". This is useful in keeping your templates short and expanding them as necessary to ensure that your notes are as extensive as they need to be, on a case-by-case basis. Here's how to do it.
- Click on Option on the question that needs to be answered a certain way.
In this example, I want to make "PBWs diagnosis notes" (the question after "PBWs taken") hidden when I leave this question blank or answer it with "Patient declined".
- Create a tag for this Question ID.
You can see that [QuestionID]=PBW. This is to identify this question in your "Hide Condition" coding in Step 4.
- Click on Option on the question to be hidden.
- Enter the coding into Hide Condition field.
This example is showing that the question "PBWs diagnosis notes" will be hidden if I do not answer the previous question "PBWs taken" (shown with coding !model.PBW) or if I answer the question "PBWs taken" with "Patient declined" (shown with coding model.PBW=='Patient declined') Please see more useful codes below.
i. ! signifies "Not".
ii. || signifies "Or"
iii. == signifies "Equals"
iv. != signifies "Not Equal To"
v. && signifies "And"
- Click Complete Template to save and exit.

Create a placeholder for a question
A placeholder is a text which appears if the textbox field has not been pre-filled. This can be used as a cue on what to type in that field. Here's how to do it.
- Click on Option on the desired question from your chosen template.
In this example, I want to change the current placeholder for the answer field of "Reason for attending".
- Enter the desired text into the Placeholder field.
- Click Complete Template to save and exit.
a) Preview: gives you an opportunity to view your template before saving.
b) Save: allows you to save your progress and continue editing.
This is what the question will look like when you'll be compiling your treatment notes.
Enter a description for a question
A description is useful if you have pre-filled a particular question and therefore you cannot see the placeholder in the field (as shown above). Here's how to do it.
- Click on Option on the desired question from your chosen template.
In this example, I want to add a description for the question "Reason for attending".
- Enter the desired text into the Description field.
- Click Complete Template to save and exit.
a) Preview: gives you an opportunity to view your template before saving.
b) Save: allows you to save your progress and continue editing.
This is what the question will look like when you'll be compiling your treatment notes.
