Overview
Core Practice has enhanced the process of securely sending and obtaining consent forms from patients. The online consent letter allows for obtaining a patient's approval for various aspects of their dental care, including treatments and financial matters.
This feature replaces the old method of using physical letters, which required manually printing and scanning documents to obtain patient signatures.

Create Form

Send an Online Consent Letter
After creating the form, the Dentist or Receptionist can use it to send the consent form to a patient. Here's how to do it:
- Go to the patient's record and click on Document.
2. Click Create Secure Letter on the left side of the screen.
3. Choose the location and provider, then click Continue to Template.
4. Look for the consent letter you want to send to the patient and click Use This Template.
5. On this screen, you can still edit the letter by clicking on the content and hitting Save.
Note: If a dentist's name and signature is required, this is where we can attach those before sending the letter to the patient.
6. When you're done editing the letter, click Send in the upper right corner of the screen.
Note: Currently, SMS is the only option for sending the consent letter to the patient.
7. You can edit the SMS message template and then click Send.
Patient Flow
- After we send the consent letter to the patient, they will receive a text message and click the link to sign the letter.
2. The patient needs to tick the box and click Next: Start Form.
3. After reviewing the letter, the patient will need to fill in the necessary information and then click Next: Accept and Submit.
4. The patient will receive a Completed message and can also view the completed document.
Clinic Flow
- After the patient submits the form, the status of the letter will change to Completed.
a) Preview: This option allows you to preview the signed document.
b) Edit: This will direct you to the certificate, timestamp, audit logs, and more.
c) Settings: This will give you the option to download the signed envelope, view evidence, and delete the envelope.
2. Under Envelopes, you can also view the Certificate, Signer, Timestamp, and Audit Logs by clicking the expand button.
