How To Send Online Forms

Overview

Forms are used to collect the required information in a logical and meaningful fashion for communication. Here at Core Practice, you can simply Print the Online Forms or send them through Email or SMS.

Steps on how to send Online Forms

Note: This method works with every form, follow the steps if you wish to Print, Email, or SMS other types of the medical form.

1. Go to Appointment Book and select Patient profile.

2. Click on More and select Complete a New Form.

3. Choose any type of Medical Form, select 3 dots, and choose either Print, Email, or SMS Form.

(A) - New Patient Medical History Form
(B) - Existing Patient Contact Update Form
(C) - Existing Patient Medical History Form
(D) - COVID-19 Clearance Form
