Online forms give an opportunity to the clinic to capture's patient data in a more efficient manner. As such, paper is no longer good enough to prove a patient's identity. Reducing the need for printing, ink, and paper, online forms offer a cheap, easy, and accessible way to collect information from your customers.
However, in some cases, patients tend to encounter an error when filling out the online medical history form specifically in the field of Health Insurance and Suburbs. Below shows an example of what you need to do to prevent at least one of any errors you may encounter.
As shown below, even though the health fund has been spelled correctly, it still comes up as an invalid name of fund/insurance.
As a suggestion, allow at least 2-3 seconds to type in letters for the suggested fund/insurance to appear from the item list.

Similarly to Health Insurance, we highly suggest allowing a quick second or 2 to type in the letters for the list of Suburbs to come up.
The purpose of this leeway is to allow the system to buffer in order to preload the required data right before you completed the word.
